Candidates seeking admission to the Diploma in General Nursing and Midwifery course should fill up the application form in a format given below and send it to -The Secretary, Dr. M.V. Shetty Memorial Trust, A.B. Shetty Circle, Mangalore -575 001,along with a demand draft for Rs. 200/- being the registration fee and Rs. 300/- towards the cost of the application which may be downloaded and filled in. Altogether a demand draft of Rs. 500/- (Rs. 200 as registration fee and Rs.300/- as application fee) drawn in favour of Dr. M.V. Shetty Memorial Trust, Mangalore , payable at Mangalore may be sent along with the filled in application form.

| Attested copies of the following certificates should be sent along with the application form.

| 1. S.S.L.C. Certificate (10th)
2. Marks cards of the qualifying examination (10+2) or both 11th and 12th
3. Pass certificate of 10+2 issued by the examination conducting authority
4. Transfer certificate issued by the College/School
5. Conduct certificate issued by the Principal
6. Date of birth in Indian Almanac (A.D.)

| Following Original Certificates should be submitted at the time of Admission with one set of attested copy.

| 1. S.S.L.C. (10th)
2. Marks cards of the qualifying exam i.e. 10+2 (both 11th and 12th Year)
3. Provisional Pass Certificate.
4. Transfer certificate issued by the college
5. Migration certificate issued by the University/Board
6. Character/conduct certificate issued by the College/School last studied
7. Physical fitness certificate issued by the Government Medical Officer
8. Pass port size photograph
9. For students from Nepal: Neplaese citizenship certificate and date of birth in Indian Almanac (A.D.)
10. For students from other country: Pass port and student visa and date of birth in Indian Almanac.
11. Eligibility certificate: Students who have completed their qualifying examinations outside Karnataka should apply for an eligibility certificate to the Rajiv Gandhi University of Health Sciences, Bangalore in a prescribed format which is given below. The filled in application form along with the .D.D. of Rs. 1000/- drawn in favour of Dr. M.V. Shetty Memorial Trust, payable at Mangalore should be sent to the office of the Trust for sending the consolidated D.D. to the University for obtaining the eligibility certificate.
12. Migration Certificate: Candidates who have passed qualifying examinations outside the University to which the course now applied for is affiliated, should obtain and produce the Migration certificate from he University/Board under which the qualifying examination was passed.
13. Transfer Certificate: Candidates who have passed the qualifying examination should produce the transfer certificate obtained from the School/College last studied.

| Rules of the Management
|
| 1. If any student, after admission, leaves the College/Course under any circumstances he/she shall pay the college fees for the remaining period of the course (Whole course)
2. All disputes arising with respect to the affairs of the College/Trust/Management shall be within the jurisdiction of Mangalore Court.
3. It is compulsory for the girl student to stay in our hostel and avail mess facilities.
4. It is compulsory for the male students to stay in our hostel.
5. It is the duty of the candidates to obtain and submit the Eligibility Certificate but however, the Management will render all assistance to get the certificates.
6. Any amount once paid to the College/Management/Trust shall not be refunded under any circumstances.
7. All admissions made are provisional and subject to approval by the University. If the University for any reason does not approve the admission at later stage, the Management is not responsible.
